E-RATE SERVICE PROVIDER FORMS FY 2008-09
**Effective November 1, 2004, all service providers who participate in the E-Rate program must have a FCC Registration Number (FCC RN). You can register for this number on the FCC web site: FCC Registration **
The following forms must be completed by all Service Providers who want to participate in the Schools & Libraries Universal Support Mechanism (E-Rate)
1. Form 498: Service Provider Information Form
This form must be completed by all service providers who participate in the E-Rate program. The form is the first form filed by Service Providers in order to receive a Service Provider Identification (SPIN) Number.

Carefully follow all directions. Note that you will be E-filing the form. The example form is Extremely helpful. Please notify us as soon as you receive your SPIN.
2. FORM 473: Service Provider Annual Certification Form (SPAC)
Every service provider who participates in the E-Rate program, must submit The Form 473 each year. SLD will NOT process invoices for a provider who does not have a current Form 473 on file.
